What is a Good Faith Estimate?

Under the No Surprises Act, healthcare providers and facilities are required to give uninsured or self-pay patients a "Good Faith Estimate" of expected charges. This estimate outlines the expected items and services, including their anticipated costs, so you can plan ahead.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you may be eligible to dispute the bill through the federal patient-provider dispute resolution process. For more information, visit cms.gov/nosurprises or call 1-800-985-3059.

How is my personal information protected?

Deer Park Physician Group, PLLC is committed to protecting your health information in compliance with the Health Insurance Portability and Accountability Act (HIPAA). Your protected health information (PHI) is used only for treatment, payment, and healthcare operations as permitted by law. We maintain administrative, technical, and physical safeguards to protect the confidentiality and integrity of your information.

Online payments are processed through Stripe, which is PCI DSS Level 1 certified. DPPG does not store credit card numbers or bank account information on its systems.

Can I dispute a charge on my statement?

Yes. If you believe there is an error on your statement or if you wish to dispute a charge, please contact our billing office in writing or by email. Include your account number, date of service, and a description of the issue. We will investigate and respond within 30 business days.

You may also file a complaint with the Texas Department of Insurance at 1-800-252-3439 or online at tdi.texas.gov if you believe your rights under the No Surprises Act have been violated.

Who is Deer Park Physician Group, PLLC?

Deer Park Physician Group, PLLC (DPPG) is a Texas-based physician-owned professional limited liability company that provides emergency physician staffing and professional medical services. DPPG contracts with emergency care facilities to ensure patients receive high-quality emergency medical care from qualified, credentialed physicians.

I received care at Community Health 1st ER but don't remember seeing a physician. Why am I being billed?

Every patient who presents to an emergency room is evaluated by a licensed physician or qualified medical provider. Even if your primary interaction felt brief, the physician conducted a medical screening examination, reviewed your medical history, ordered and interpreted diagnostic tests, made clinical decisions regarding your care, and formulated a treatment or disposition plan. These constitute professional medical services for which the physician is entitled to bill.

I already paid the emergency room. Why am I receiving another bill?

The payment you made at the emergency room was likely for facility services billed by Community Health 1st ER. As explained above, physician professional services are billed separately by Deer Park Physician Group, PLLC. These are two distinct charges for two distinct categories of service, and both are standard components of an emergency room visit.

How do I request my medical records?

Medical records from your emergency room visit are maintained by Community Health 1st ER. To request copies of your medical records, please contact the facility directly. DPPG can provide billing records and documentation related to the professional services charge upon written request.
